Because the Cloak is so recognizable as part of the accouterments of the Sorcerer Supreme, some may say iconic, it's no surprise to Dr. Strange that someone in this bizarre place might recognize him. When he turns to see who is calling after him-- his face will look familiar to Gwen, although not as the Strange who helped her in the past. (He does look startlingly like Buttercup Cumberpatch.)

It occurs to him slowly that when she called for him to get his attention, he could not have ignored the summons if he had tried. Stephen had taken great care to seal his mind from invasion, but this creature seems to be a little more persistent than most.
